Transcranial Doppler and Cortical Microcirculation at Increased Intracranial Pressure and during the Cushing Response

The effect of increased intracranial pressure on the flow velocity of the basilar artery was measured with transcranial ultrasonic Doppler in New Zealand White rabbits under alpha-chloralose anesthesia and artificial respiration. Laser Doppler flowmetry served to study changes of the cortical microcirculation. The results confirm a high inverse correlation of the diastolic flow velocity, the pulsatility index, and the resistance index with the cerebral perfusion pressure (CPP). During acute intracranial hypertension, however, these parameters do not show a good correlation with the local cortical blood flow. Background Airway diseases are highly prevalent in infants and cause significant morbidity. We aimed to determine the incidence and risk factors for respiratory morbidity in a Spanish cohort of moderate-to-late preterm (MLP) infants prospectively followed during their first year of life. Methods SAREPREM is a multicenter, prospective, longitudinal study. PURPOSE In this animal study, density ranges for CT-based quantification of ventilated lung area were determined. Healthy lungs and ARDS lungs were compared during artificial respiration. MATERIAL AND METHODS CT-scans were performed in 5 anesthetized pigs using a dynamic multiscan CT option on a predefined transverse slice (slice thickness 1 mm; effective temporal resolution, 250 ms). During continuous CT acquisition, airway pressure was increased or decreased in a stepwise manner. In all images, areas of defined HU ranges were determined planimetrically. Pressure-Induced Growth (PIG) of Atretic Esophagus: A Contigent Management for High-Risk Esophageal Atresia

The management of uncomplicated esophageal atresia is a straightforward procedure consisting of thoracotomy with anastomosis of the atretic segments and ligature of any tracheoesophageal fistula present. However, besides the rareness of isolated esophageal atresia, our observations in the past few years show increasing incidence of the anomaly in association with premature births and other malformations which are usually incompatible with life. Aspiration pneumonia resulting from reflux of gastric content through a lower tracheoesophageal fistula, rather than the overflow of saliva from an upper atretic pouch, also threatens the life of an otherwise healthy neonate with esophageal atresia.
